Health System Reform in Practice: Models that Support and Sustain Care 

Health system reform often keeps caregivers on the sidelines, despite their central role in sustaining care. Reform demands models that can be implemented and sustained. This session brings together leaders who have driven change in four critical areas: embedding caregiver voices in care design, strengthening community supports and integrating with health systems, advancing physician leadership in oncology and chronic condition management, and integrating underserved groups. Panelists will detail what worked, what barriers remain, and how reform can be scaled. Attendees will leave with practical examples of policies, programs, and delivery models that demonstrate how health system reform can move from concept to reality.
